As per the fourth Health Index launched by Niti Aayog and prepared in collaboration with the Ministry of Health and Family Welfare with technical assistance from the World Bank, Kerala is ranking topmost amongst larger states, while Uttar Pradesh is ranking worst in overall health performance. Tamil Nadu and Telangana are in second and third positions, respectively. The reference period of the Index is 2019-20.

In terms of incremental performance, however, Uttar Pradesh is ranked at the top. The state has registered the highest incremental change from the base year 2018-19 to the reference year 2019-20.

Amongst the smaller states, Mizoram has emerged as the best performer in terms of both overall and incremental performance. Among UTs, though Delhi and Jammu and Kashmir rank among the bottom in terms of overall performance, they are leading in terms of incremental performance.
